When Agent Cowles first meets John Clancy, the case files she puts down on his kitchen surface are seen in one side shot, and then have disappeared by the next shot.

Continuity
In the train when John Clancy gets shot by Charles Ambrose you can see that the bullet hit him right at the top of the ear, in the next shot, at the hospital, the scar left from the bullet is on his forehead.

Crew/Equipment Visible
When the car chase is happening and the taxi gets in that intersection, you can see the chain that is pulling the car in front of that jeep, and also after when Agent Katherine's car flips over, it can easily be seen Dr. Clancy's stunt doll instead of him, when it's back on its wheels.
Trivia:
The script for this movie was originally picked up by New Line Cinema, with the intention to rewrite it as a sequel to Se7en, tentatively titled 'Ei8ht'.

After spending close to two years in the Warner Bros. vault, the film will be released in 2015, courtesy of Relativity Media - who have bought the picture from Warner.

The project was originally going to be directed by Shekhar Kapur with Bruce Willis in the lead role.
